Release Form (conference room) at DVinfo.net

Does anyone have a release form I could use that I could put on the outside of a conference room (on the doors or wall when entering) that states that anyone who enters the room will be subject to filming.

I found this, is it enough?

A video is being recorded on the premises.
By entering the premises, you agree
to allow your likeness and /or voice
to be used in connection with the
production of the video.

Your goal is to reduce the likelihood that someone will complain, make a fuss and/or try to interfere with the distribution of your production. Clearly indicating what the expectations are is a huge step toward limiting that.
