Looking for basic Contract/form for Wedding Videographer at DVinfo.net

I'll be opening up my business soon and I was wondering if anyone could share a very basic contract or information form they give to the clients. I'm looking for ideas and things to include on my own contract or somthing to help me get started. I'm also willing to pay for help.

I'm also looking for somthing like a Wedding Info Sheet. I mean I have a god idea on what I want. I just wanted to make sure I wasn't missing anything.
