need quote form at DVinfo.net

Well, I'm stuck in Kelowna for a few days, and need to get a proposal done for a client. However, I'm without my usual tools (crayons, glue, macaroni noodles, sparkles), so I'm hoping someone here can e-mail me a quote form. Something with a breakdown of potential expenses/costs/rentals, etc... to base a quote on. I have one at home, but no-one is there to send it to me. If anyone uses something like this, could you please email it to me at the_klenzer@hotmail.com preferably as text pasted into the body of the message.
