Looking for - login, watch video, e-sign system at DVinfo.net

Does anyone know of a hosted solution where you can request a person to watch a video and then when they have watched it to the end they e-sign to say that they have watched it and understood it.

Looking (and failing) to find a system to host a heath and safety video which could either be sent as an email invite or filled out on-site for contractors. This must exist?

I believe Jotform offers e-sign. If it's a part of a webpage you could embed the video onto the page, then embed the jotform below it.
