Brian,

1) XLR cables for your shotgun:
I had mine custom-made by Kindcables,
about 1 foot long, with two right-angle XLR connectors
(you can modify the "direction" as you please)
Check'em out on the web. I think they're out west (Colorado?)
Or just drop by your nearest audio specialist...

2) I strongly suggest you forget the camera mic holder,
and get yourself a shockmount.
I use this one:
Azden | SMH-1 Shock Mount for Shotgun Mics | SMH-1 | B&H Photo
it's cheap and it works, but there are many others out there...

3) If you need more hotshoes, check out the J-rod (twin mount):

I also recomend a shock mount. The one listed above is good, but most brands are at or under $49 at BH. I got my Rode as part of a boom kit. But any brand is good. Eliminates all vibration noise from the camera and tripod. No more hearing your hands clicking levers or servos grinding away. Never leave home without it.
