Quit breathing on me!

My XL2 has picked up a new habbit of not staying sharp on me. On maual focus I go in for my rack focus on the eyes etc.. I pull out and it is as fuzzy as can be. Looks like the back focus is out but how do I adjust? Thoughts?

Where can I send this to be fixed in Dallas?
