Follow Focus - anyone find a mitre/worm gear that works well?

I know there has been alot of talk about the Follow Focus and the delicate ratios and so forth to get it working perfectly (most of which I don't understand), but I was wondering if anyone has found a worm/mitre gear that works despite all the complications involved - hopefully something inexpensive. Url would be great as well!
