Does someone know where I could find a form for shot logging? Maybe this isn't the right term. What I mean is, let's say you're shooting a movie, and you would have a form on which to mark your takes, what scene you're shooting, etc., plus a place to mark down which takes are good or bad and why.

I can make my own if I have to, but if there's an already-created form of this type, that's what I'm looking for.

Also, if there's some website or something with information on how to be a script supervisor, and maybe some basic cinematography tips, like what'd you'd find in "The Five C's of Cinematography." I'd like a site I could refer people to.

I think it's better if you make your own take sheets because you can space things out the way you want. We organize ours with spaces for: scene number, in timecede, out timecode, take number and whether good, bad or questionable, and space for comments that needs to be bigger than the other spaces. Most forms I've seen have the lines too close together. It's much easier to edit when the spaces are bigger.
