After trying several options, I have settled on Rycote Universal Kit mount, attached with a short swing arm to the hot shoe. (I have removed the Canon microphone mount.) The Rycote unit provides excellent isolation. I also use a Remote Audio Shortie coiled XLR cable with a right-angle connector.

For the microphone, I am using a Rode NTG-1 with phantom power.
