AbelCineTech just started selling the 5D Mark II and Canon lenses. So of course I've put together a Arri mattebox kit for it. It's the MMB-1 with a custom riser, belows back, and step-down/up rings. Check out the video on our blog -

Andy any ballpark figure for cost on this?

Fine for tripod, but for shoulder mount, you will need to get get camera offset, with a a viewfinder magnifier and longer rails to mount shoulder pad... something like this:

Admittedly a bit dated and low budget, but this is general idea where a total system should go.

The kit is approximately $2,400. It's well suited for a tripod, but I'd also use the MMB-1 with a handheld rig with the proper offset.
